Input wanted for community health assessment in Phillips Co.

The Northeast Colorado Health Department is currently conducting a Community Health Assessment that will direct our services focus for the next five years. This two-part assessment analyzes a number of community health indicators including risk factors, quality of life, mortality, morbidity, community assets and social determinants of health and health inequity. In addition, information will be gathered on how effectively the public health system provides essential services. The data from this Community Health Assessment will inform decision-making, the prioritization of health problems and the development, implementation and evaluation of our next public health improvement plan.

The NCHD would like community members to help conduct this assessment to best serve Phillips County in the coming years. There are two ways to provide input.

The first way is to attend a Community Engagement Meeting. There will be Community Engagement meetings around the region over the next several weeks. In Phillips County, a community meeting will be held at the Phillips County Event Center, 22505 US-385, Holyoke on Tuesday, Oct. 25 from 1-3 p.m. This meeting will be held in a hybrid fashion so you can attend in-person or via zoom. Community members are encouraged to attend this meeting and engage about the needs of our community.

In addition to the community engagement meetings, NCHD is conducting a survey to evaluate the needs of the communities. This short survey should take about 10 minutes to complete and gives insight on the needs of each community. The survey is available in English, Spanish or Somali and can be accessed on our website (www.nchd.org). Or if you prefer, a hard copy of the survey at one of our NCHD offices.

If this date and time does not work well for your schedule, a complete list of community meetings can be found on our website. To participate via zoom, please register at our website and a zoom link will be sent to you.

The NCHD would like to invite all members of the community to take part in this health assessment. Furthermore, if you know of others that may be interested in participating as well, please share this information with them.
